The inspector’s account

On 11/07/2025, at approximately 10:00am, an office meeting was held to discuss Complaint 11-AS-20200420145538. Present at the meeting was Eva Alvarez, Licensing Program Manager (LPA), Alfonso Iniguez, Licensing Program Analyst (LPA), Aaron Khodorkovsky-Administrator and Steven Atlas-Licensee.

During the meeting, LPM Alvarez reviewed the details of the Complaint. On May 5, 2022, the Department substantiated an allegation: Facility staff hit residents in care

At the time the findings were delivered on May 5, 2022, the Department indicated that an enhanced civil penalty determination was pending, pursuant to Health and Safety Code Section 1548(f)(1)(A).

The Department is reviewing the complaint for enhanced civil penalty for physical abuse pursuant to H&S 1548(f)(1)(A). The total amount for the civil penalty totals $10,000 for physical abuse.

An exit interview was conducted, and a copy of this report was provided together with the appeal rights.
