Facility Evaluation Report
Licensing Program Analyst (LPA) Glenn Trueman conducted an unannounced required annual inspection visit. LPA met with Staff Jacob Lee and explained the reason for the visit. The facility is approved for four (4) adults ages 18-59, ambulatory only. Clients are serviced by Frank D Lanterman Regional Center.
LPA used the Compliance & Regulatory Enforcement Tool (CARE) during today’s inspection. LPA Trueman toured the facility with assistance of Staff Jacob Lee.
The facility is in a residential area and is made up four (4) client bedrooms, two (2) staff bedroom, two (2) full client bathrooms and one (1) full staff bathroom, kitchen, dining room, living room, laundry located next to kitchen, front and backyard. There was no body of water observed in the property.
Deficiency was observed in 1 client's bedroom in which there were numerous cracks in the wall and was in need of repair.
LPA reviewed four (4) out of four (4) client files which consisted of Admission Agreements, Physicians Report, Consent Report, Individual Program Plan, and Centrally Store Medication for one (1) client only. LPA reviewed three (3) staff files which consisted of Personnel Report, Health Screening, Criminal Record Statements, Fingerprint Clearance, First Aid and CPR.
Deficiency was observed upon review in which First Aid and CPR for the Administrator was expired.
