Facility Evaluation Report
On November 20, 2024, Licensing Program Analyst (LPA) Deborah Lee conducted an unannounced required annual visit using the CARE Inspection Tools. LPA met with Nelly Gomez and Roca Amir and explained the purpose of this visit. The facility is licensed to operate for four (4) ambulatory clients nages 18 through 59 from South Central Los Angeles Regional Center. Currently, there are only (2) clients living at this facility.
The facility is a single-story structure located in a residential neighborhood. It consists of the following: two (2) client's rooms, two (2) common bathrooms, living area, dining area, kitchen, laundry, den, and outside area. LPA toured the physical plant. There were no bodies of water or obstructions on the premises. All rooms were inspected. Beds and bedding supplies were in good condition, adequate lighting was provided,
Physical Plant LPA and Lead DSP staff toured the facility inside and outside. LPA observed there were no bodies of water on the premises. All rooms were inspected. Beds and bedding supplies were in operational condition, lighting was provided, and storage for the client’s personal belongings was observed. LPA observed that facility had required postings: Facility license, personal rights, facility sketch, exit signs, infectious disease postings, Administrator certificates, an emergency disaster plan.
