Facility Evaluation Report

On 06/12/2025, Licensing Program Analyst (LPA) Antonine Richard conducted an unannounced annual required visit using the New Inspection Tools. LPA Richard met with Licensee Marvin Cutchins and explained the purpose of today's visit was to conduct the facility's annual inspection and review the facility's physical plant, medications, food service, clients, and staff records.

LPA Richard verified all current staff fingerprints are cleared/associated with the facility. Client records were checked. Staff files were reviewed. The facility currently has three (3) ambulatory South Central Los Angeles Regional Center (SCLARC) consumers in placement.

The facility is a single-story family home located in a residential neighborhood. Licensee Cutchins and LPA Richard toured the facility which included: Living room, dining room, kitchen, breakfast nook, 3 bedrooms, 2 bathrooms, laundry room, storage room, detached garage, and indoor/outdoor activity areas.
