Facility Evaluation Report

On 06/06/25, Licensing Program Analyst (LPA) Elvira Gonzalez conducted an unannounced annual required visit using the CARE Inspection Tool. LPA met with Licensee, Eyob Wallano, and explained the purpose of today’s visit. LPA was granted access to the facility. The facility is licensed to serve (4) developmentally disabled clients ages 18-59. The facility is approved for (4) non-ambulatory clients. Currently there are two (2) clients in placement.

The facility is a single-story home located in a residential neighborhood. It consists of the following: two (2) client bedrooms, of which only one is being used at the moment, two (2) staff bedrooms, one (1) client bathroom, one (1) staff bathroom, a living room, dining room, kitchen, and a detached garage.

LPA Gonzalez and Eyob Wallano toured the entire physical plant. There were no bodies of water or obstructions on the premises. Exits/ Walkways around the home were free of debris and hazards. All client bedrooms were inspected.
