The inspector’s account

LPA interviewed clients, all four (4) clients interviewed stated they did not have knowledge about the allegation. All four (4) staff interviewed were corroborated the allegation. Per staff interviews, staff revealed facility did not provide proper annual in- service training to the current staff. Per file review, it revealed that staff had sign in for training; however, staff showed they could not recollect the training materials and had discrepancies on the information as claimed in the records. Thus, administrator did not provide training to staff per regulations which may due to lack of ability and qualification.

In regard to allegation of facility failed to train staff adequately, it was alleged that staff were not proper trained per Title 22 regulations. LPA interviewed clients, all four (4) clients interviewed stated they did not have knowledge about the allegation. All four (4) staff interviewed were corroborated the allegation. As mentioned above, staff interviews revealed facility did not provide proper annual in- service training to the current staff. Staff could not recollect the training materials and had discrepancies on the information as claimed in the records. Thus, facility failed to train staff adequately.

In regard to allegation of facility falsified staff training records, it was alleged facility created staff’s training documents which training were not conducted. LPA interviewed clients, all four (4) clients interviewed stated they did not have knowledge about the allegation. All four (4) staff interviewed were corroborated the allegation. Per file review, it revealed that staff had sign in for training; however, staff could not confirm whether the training was conducted as claimed in the records. Thus, facility falsified staff training records.

Based on LPA's observations, record reviews and interviews conducted the preponderance of evidence standard has been met, therefore the above allegation is found SUBSTANTIATED.

Deficiencies are being cited according to California Code of Regulations, Title 22, Division 6, Chapter 8 on LIC 9099D.

An exit interview was conducted with Staff#2, lead staff. A hard copy of this reports and appeal right were provided.
