The inspector’s account

Licensing Program Analyst (LPA) Sanjay Vaid conducted 24-hour Case Management visit to the facility and met with Josie Rojas and conducted a Health and Safety check the facilities three levels with Rojas and did not observe any deficiency during the tour.

Program Administrator Ray Tse arrived shortly after and discussed the reason of the visit.

LPA Vaid requested personnel file for staff S1 and client file for client C1. Administrator Ray Tse requested the regulations for releasing documents. LPA Vaid provided regulations 81066(e) personnel files /81070 (d) client files.

Staff (S1) had alleged sex with Client (C1) while working at the facility, over two years ago.

Client (C1) files obtained -face sheet, physicians report, Individual Program Progression notes, needs and service, medications list, psych evaluations.

Personal files staff (S1) obtained- staff contact information.

LPA Vaid provided copy of this report to Administrator Ray Tse.
