Facility Evaluation Report
Licensing Program Analyst (LPA) Alberto Lopez conducted a required unannounced annual inspection visit. LPA met Licensee Daniel Tabello. The home is licensed as an Adult Residential Facility to serve 6 ambulatory clients in the age range of 18 through 59. Currently there are no clients in care. The fire clearance has been approved for six (6) Ambulatory Clients. The physical plant was toured with the Daniel Tabello, Licensee
The facility is in a residential area and is a two-story family home. The first floor consists of a living room, one (1) bathroom, dining area and kitchen. The second floor consists of four (4) bedrooms of which two are shared, two (2) restrooms and a laundry room. LPA conducted the tour and observed the following: Smoke/carbon monoxide detectors were observed in common areas and in each client’s bedroom and were tested. Client’s bedrooms were observed to have required furniture such as bed frames, dressers, chairs, lamps, and sufficient closet space. The required bedding was also observed. The home has three (3) bathrooms and is clean and sanitary. The water temperate was tested and measured at the required 105-120 degrees F. There is a fire extinguisher in the kitchen and garage. All appliances in the kitchen were observed to be clean and operational. The sharp knives are in the kitchen cabinet that is inaccessible to clients along with cleaning supplies and chemicals. Medications will be locked in a kitchen cabinet. All files will be kept in the cabinet in the garage. The garage will be used to store locked chemicals and an extra refrigerator for extra food. The backyard has shaded area with patio furniture. The home does not have a pool or any large bodies of water. There are cameras only in common areas inside the home and outside. No client records were reviewed since the home has no clients as of today’s visit.
No deficiencies observed today. Technical advisories issued and exit interview was conducted with Licensee and copy of this report provided.
