Facility Evaluation Report
Licensing Program Analyst (LPA) Gabriela Castro conducted an unannounced required annual inspection using the Compliance and Regulatory Enforcement (CARE) Tool. LPA was greeted by staff, Maryann Osoluka, and explained the purpose of the visit. Administrator, Shenisia Gladney, was contacted by telephone and was available throughout the inspection.
The facility is licensed to provide care and supervision for four (4) adults, ages 18 to 59, with developmental disabilities, of whom two (2) may be non-ambulatory. At the time of the visit, all three (3) clients residing at the facility were receiving case management services through the South Central Regional Center.
Personal Rights postings were observed in a common area. Residents had access to personal space, privacy, and adequate storage. No firearms/weapons were present.
The facility is a one-story residential home consisting of two client bedrooms, one storage room, a kitchen and dining area, living room, two bathrooms which one is a staff restroom, a laundry room, and a backyard patio with a storage shed. During the visit, the LPA observed that both client bedrooms were equipped with the required furnishings, including a bed with a clean mattress, clean linens, a dresser, a chair, and adequate lighting. Extra clean linens and towels were stored in cabinets located in the laundry room. All furniture was observed to be clean and in good repair. Both bathrooms were equipped with functioning sinks, toilets, and showers, and contained the required non-skid mats and grab bars. The hot water temperature was tested and measured within the regulatory range of 105°F to 120°F.Smoke detectors and carbon monoxide detectors were tested and found to be operational. A fully charged fire extinguisher was mounted in the kitchen. All passageways and exits were clear and unobstructed, and no bodies of water were observed on the premises. The backyard provided shaded seating for residents, and an adequate supply of emergency food and water was observed. Cleaning supplies and chemicals were stored in locked cabinets in the laundry room, making them inaccessible to residents. Sharp objects were also secured in a locked kitchen cabinet.
