Facility Evaluation Report
On 06/12/25, Licensing Program Analyst (LPA) Yolanda Rosser arrived at facility to conduct an unannounced annual required visit using the CARE Inspection Tool. LPA was provided entrance into the facility by DSP, Rodolfo Bello. LPA met with Sarita Wellington, Assistant Administrator and the purpose of today’s visit was explained. The facility is licensed to operate for (4) ambulatory, developmentally disabled adults ages 18 through 59. Census is currently (4) clients.
The facility is a one-story home located in a residential neighborhood. The property consists of the following: 3 client bedrooms, 2 common bathrooms, staff bedroom, living room, kitchen, dining room, detached garage and an outdoor shaded area with plenty of seating. There is a schedule of activities and exercise for residents posted. Menu is posted on refrigerator. There is a extra refrigerator and freezer in the garage with extra food supply. There is a closet located in the hallway with extra linen and towels, and toiletries. The house is clean and bright and airy.
LPA conducted a records review of (4) client records, (5) staff records and reviewed the facility disaster plan. All client & staff records were complete. The facility disaster plan was current and in compliance with Title 22 at the time of visit.
LPA reviewed (4) Client Medication Administration Records and did not observe any discrepancies at the time of visit.
