The inspector’s account

On 08/22/20254 at approximately 02:00PM Licensing Program Analyst (LPA) Troy Watson conducted an unannounced annual required visit using the CARE Inspection Tool. LPA met with DSP Arli Usher and the purpose of today’s visit was explained. The facility is licensed to operate for (6) ambulatory, (0) non-ambulatory, (0) bedridden (developmentally disabled or Mentally Ill) adults ages 18 through 59. The facility has an approved Hospice Waiver for (0) clients. Currently, the home has (0) clients. The clients are Westside Regional Center/ south Central Los Angeles Regional Center/ Harbor Regional Center/ Department of Mental Health) clients. None of the clients have Restricted Health Care Conditions and none utilize postural support or protective devices.

The facility is a two-story home located in a residential neighborhood. The property consists of the following: 5 client bedrooms, 2 common bathrooms, a staff office, living room, kitchen, dining room, detached garage, and an outdoor shaded area. LPA conducted a records review of (4) client records (6) staff records and the facility disaster plan All client & Staff records were complete. The facility disaster plan was current and in compliance with Title 22 at the time of visit. LPA reviewed (4) Client Medication Administration Records and did not observe any discrepancies at the time of visit.

On 08/22/2025 at approximately 02:10 PM LPA Watson and DSP Arli Usher toured the inside and outside of the facility. All client rooms were checked. Mattresses and box springs were in good condition, adequate lighting was observed, plenty of dresser and closet space was observed. Walls and floors were clean and in good repair. Bed linens, comforters were checked, and bath towels were adequately stocked at the time of the visit. Bathrooms were found to be within Title 22 regulations.
