Complaint Investigation Report
The investigation revealed the following: One (1) out of six (6) clients interviewed stated drinking water was only in the kitchen and client did not access to drinking water a few hours daily. Five (5) out of six (6) clients interviewed revealed that drinking water are accessible to clients throughout the day located at dining area in the facility. Juice, water, and other beverages were provided during mealtimes. Med room also has drinking water available to clients. All four (4) staff who were interviewed denied the allegation. Staff interviews revealed facility had a hydration policy to ensure clients have access to drinking water and stay hydrated. LPA toured the facility and observed two drinking water jugs located at the dining room are full and available to clients. Therefore, drinking water is available to clients to stay hydrated throughout the day at the facility.
In regard of the allegation, “staff do not allow residents to perform their own hygiene and exercise in the facility,” it was alleged that facility did not allow client#1 to shave and exercise at the facility. The investigation revealed the following: interviewed with client#1, client said staff did not allow client to shave and staff would watch client when client was shaving and collect the razor right after. Client#1 stated staff did not allow client to exercise in the stairways in the facility. Per file review, client#1 has a history of losing razors and attempt suicide. Five (5) out of six (6) clients interviewed revealed that hygiene supplies were provided to clients three times a week for shaving and clients were allowed to exercise at the facility at their own will. Clients could go to facility gym if needed. All four (4) staff who were interviewed denied the allegation. LPA tour the facility with Administrator, LPA observed clients were going around or taking stairs at the facility. Therefore, clients could perform their own hygiene and exercise in the facility.
In regard of the allegation, “staff are discriminating against client,” it was alleged that facility staff discriminated against client#1. The investigation revealed the following: interviewed with client#1, client said staff discriminated against client. Five (5) out of six (6) clients interviewed revealed that staff did not discriminate clients. All four (4) staff who were interviewed denied the allegation. LPA tour the facility during resident interview, LPA observed staff treated clients nicely. Therefore, there is not a preponderance evidence to prove the facility staff discriminate against client.
Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore, the allegations is UNSUBSTANTIATED.
