Facility Evaluation Report

Licensing Program Analysts (LPAs) Cynthia Chan and Elena Mallett conducted a case management visit due to deficiency observed during a complaint investigation. LPAs met with Beth Welch, the Program Director, and explained the reason for this report.

During the complaint investigation today, LPAs reviewed the medications for 5 Clients. Client #1 was not given the bedtime medication Benztropine as prescribed by the physician. The electronic Medication Administration Record indicated that the medication was not available for several days in June. However, Staff found the medication which was misplaced and observed the medication was not given daily.

Based on record review, Client #1 was not given all the medications as prescribed. A deficiency is issued on the LIC809D. An exit interview was held and a copy of this report was given to the Program Director.
