The inspector’s account

The investigation reveals: That staff are not providing adequate food service for the residents.” It is alleged that the facility does not comply with C1’s dietary restrictions. The facility does not have a formal kitchen but has a dining room. All foods are prepared at their main site, and the food trays are brought over. According to the Program Director, when C1 was admitted into the facility, they stated they were lactose intolerant. The Program Director and all staff stated that there have been times when the main site has sent regular food trays to C1, and, when observed, the facility has offered alternatives. They further stated that C1 has consistently refused the alternatives and wants food delivered, which is against the facility's policy. Staff have also stated that they have observed C1 eating diary on numerous occasions. 1 out of 7 clients stated the food issue has been corrected, and on three (3) occasions, the main kitchen has sent trays with dairy. C1 also stated that they have eaten dairy, such as ice cream. 2 out of 7 clients stated that the facility follows their dietary restrictions and that they have no issues with the food. LPA conducted a file review and confirmed that C1’s physician’s report, functional and capability assessment, pre-placement appraisal, and medical assessment from Los Angeles Health Centers confirmed that C1 does not have any dietary restrictions.

Based on LPA's interviews, the investigation revealed that although the allegation may have occurred or is valid, there is not a preponderance of evidence to prove whether the alleged violation occurred; the allegation is UNSUBSTANTIATED.

An exit interview was conducted with Program Director Vanessa Reyes, and a copy of this record was provided.
