Facility Evaluation Report
On 09/03/25, at 9:15am, Licensing Program Analyst (LPA) Gina Saucedo conducted an unannounced case management visit at this facility for the purpose to ensure Health and Safety. LPA met with Brian Rosales, Administrator and explained the purpose of the visit.
During this visit, LPA obtained the following information:
2. Conducted a Physical Tour-There was two (2) staff providing care and supervision.
3. LPA reviewed and observed the medication cart that is shared for both facilities. The medication system the facility uses is called Yardi EHR.
Let it be noted, there is one (1) telephone line for both facilities.
No immediate health and safety issues observed. Exist interview conducted and a copy of this report was given to the Brian Rosales, Administrator.
