Facility Evaluation Report
Licensing Program Analyst (LPA) Raymond Comer conducted a Case Management (CM) visit to the facility to follow up on a facility building alteration made by the licensee. LPA met with Los Angeles Fire Inspector, Herbert Reddick, facility administrator Brain Robles, and advised Administrator of the visit.
Today's CM visit consisted of a physical plant inspection and Inspector Reddick's assessment of the altered interior wall. Community Care Licensing-Woodland Hills Office received information that licensee made an un-permitted alteration to the interior wall separating Facility#197607979 (licensed for a capacity of 4; current census is 2) and Facility#197607055 (licensed for a capacity of 5; current census is 3)
Deficiency cited; LPA requested for the licensee to submit the following in order to comply:
· Los Angeles Department of Building and Safety (LADBS) permit allowing the wall's alteration, while maintaining the proper fire resistive rating, OR, to have the wall restored to its original condition at the time of that the facility fire clearance (STD 850) was approved and issued.
