On 09/10/25, at 8:55am, Licensing Program Analyst (LPA) Gina Saucedo conducted an unannounced case management visit at this facility for the purpose to ensure Health and Safety.
LPA met with Daniela Blanco, Caregiver and explained the purpose of the visit.
During this visit, LPA obtained the following information:
2. Conducted a Physical Tour-bathrooms, bedrooms and living room area.
3. There were two (2) staff providing care and supervision.
No immediate health and safety issues observed. Exist interview conducted and a copy of this report was given to the Daniela Blanco, Caregiver.
