Facility Evaluation Report

On May 30, 2025, at 1:00 PM, an office meeting was held via telephone with Licensee Kenneth Watts. In attendance were Regional Manager (RM) Benita Yates, Licensing Program Manager (LPM) Stephanie Cifuentes, and Licensing Program Analyst (LPA) Elvira Gonzalez. The purpose of this meeting was to discuss the status of the license/partnership.

During the meeting, Kenneth Watts informed RM, LPM, and LPA that his partner/Licensee Fernando Cover was no longer part of the business and has not been for the last three years. Kenneth Watts stated that his clients are coming from hospital emergency rooms on a temporary basis.

RO informed Kenneth Watts that the license has been forfeited because Licensee Fernando Cover is no longer part of the business. RO informed Kenneth Watts that he will need to decide if Fernando Cover will continue with partnership, submit a Change of Ownership Application or proceed with closure.

Kenneth Watts agreed to contact LPA Gonzalez with his decision by June 2, 2025.

An exit interview was conducted with Kenneth Watts, and a copy of the report was provided.
