The inspector’s account

On 05/23/25, Licensing Program Analyst (LPA) Yolanda Rosser conducted an unannounced annual required visit using the CARE Inspection Tool. LPA met with the DSP, Jason Darensburg and the purpose of today’s visit was explained. Entrance was granted into the facility. The facility is licensed to operate for (4) ambulatory developmentally disabled adults ages 18 through 59. Currently, the home has (4) clients. The clients are Westside Regional Center clients. None of the clients have Restricted Health Care Conditions, and none are utilizing postural supports or protective devices. Later LPA met with Manager, Lonnie Turner.

The facility is a single family home located in a residential neighborhood. The property consists of the an outdoor patio with a shaded area. LPA observed BBQ grill, exercise equipment and extra outdoor seating area. The lawn is manicured and well maintained. The home is bright and airy. The home has ample seating inside and outside the home for residents use and enjoyment.

LPA Rosser conducted a records review of (3) client records, (3) staff records, reviewed the facility disaster plan. All client & Staff records were complete. The facility disaster plan was current and in compliance with Title 22 at the time of visit. LPA reviewed (4) Client Medication Administration Records and did not observe any discrepancies at the time of this visit.

LPA Rosser and Lonnie Turner toured the inside and outside of the facility. All client rooms were checked. Mattresses and box springs were inspected and in good condition, adequate lighting was observed, plenty of dresser and closet space was observed. Walls and floors were clean and in good repair. Bed linens, comforters and bath towels were adequately stocked at the time of visit. Restrooms were found to be within Title 22 regulation. Toilets and water faucets functioned properly. Shower was free of mold/mildew, there is adequate lighting, and sufficient toiletries provided for the clients use. The water temperature properly measured between 105F - 120F). CONT..........809C
