The inspector’s account

On 10/16/2025, Licensing Program Analyst (LPA) Bernadette Allen conducted an unannounced visit to conduct an annual inspection. Upon arrival LPA introduced herself to Alefian Fnu- Support Staff and he was informed of the purpose of my visit and allowed LPA entry into the facility.

At 12:15 PM, LPA reviewed three (3) staff files for First Aid/CPR certification, criminal record clearance, training's, and health screenings, which all appeared to be current.

LPA reviewed three (3) residents’ files for admission agreements, updated physician reports, and needs and services plans which appeared to be current.

At 1:25 PM, LPA toured residents’ bedrooms, and they all have the required furniture including beds, dressers, and nightstand, bedding supplies, which were in good condition, adequate lighting, and storage for resident’s personal belongings. Bed linens, comforters, and bath towels were adequately stocked at the time of visit.

Bathrooms appeared to be clean and operational. All showers have secured safety handrails and nonskid material and there are storage areas for personal hygiene items. The water temperature measured between 105-120 degrees Fahrenheit.

LPA conducted an inspection of the kitchen and there LPA observed a 5-day supply of perishable foods and a 7-day supply of nonperishable foods. All sharps and knives are locked and secured in cabinet. All cleaning supplies were inaccessible to residents.
