Facility Evaluation Report
On 05/09/2025 Licensing Program Analyst (LPA) Yolanda Rosser conducted an unannounced Annual inspection visit. LPA Rosser met with Administrator, Barbara Kizzie and explained the purpose of today's visit. The facility is licensed to support 55 ambulatory adults, of which eight (8) can be non-ambulatory. The placement agency for the clients served at this facility are with West Side Regional Center and South Central Regional Center.
The staff to client ratio is one (1) staff to three (3) clients. The last fire inspection was conducted by the Los Angeles Fire Department on 05/07/2025. The program does not provide transportation. LPA, Rosser and Administrator Barbara Kizzie conducted the tour of the facility.
The physical plant consists of (3) activity rooms, kitchenette, (3) restrooms, (4) secured storage rooms, (2) office spaces, an indoor ramp located at in the rear entrance/exit, and an outdoor parking area. (1) restroom accommodates non-ambulatory clients. Smoke detectors were tested and were operational throughout the facility. The Carbon monoxide detector was tested and operational. LPA observed (3) fire extinguisher's that are accessible for use and fully charged throughout the facility. A microwave, refrigerator, washing machine and dryer are available for use. The facility has multiple storage shelves and labeled with clients names for personal use. LPA tested the water temperature was measured and is within the required range
(107.2 - 110 F). LPA observed chemicals stored in locked cabinets and storage rooms.
