Facility Evaluation Report
On 11/14/25 Licensing Program Analyst (LPA) Elvira Gonzalez conducted an unannounced annual required visit using the CARE Inspection Tool. LPA met with Administrator, Ebony Brown, and the purpose of the visit was explained. LPA was granted access into the facility. The facility is licensed to operate for six (6) ambulatory developmentally disabled adults ages 18 through 59 years. Currently, there are three (3) clients in placement.
The facility is a single-story structure located in a residential neighborhood. It consists of the following: three (3) bedrooms, two (2) bathrooms, living room, dining room, kitchen, office area, a detached garage with laundry area, and an outdoor shaded area.
LPA and Administrator Ebony Brown toured the physical plant. There were no bodies of water or obstructions on the premises. All rooms were inspected. Beds and bedding supplies were in adequate condition, lighting was provided, and storage for the client's personal belongings was observed. Bed linens, comforters, and bath towels were stocked during the visit. The bathrooms were found to be within Title 22 regulation. Water temperature properly measured between 105F-120F. A comfortable temperature was maintained in the facility.
LPA observed the facility to be furnished at the time of the visit. Storage areas for personal hygiene were stored and not accessible to clients. The kitchen was inspected, and sufficient perishable and non-perishable food was maintained adequately. A fire extinguisher was fully charged, smoke detectors and carbon monoxide were operable. A review of Medication Administration Records was maintained in order and accurate.
