Facility Evaluation Report
On 05/22/25 Licensing Program Analyst (LPA), Yolanda Rosser conducted an unannounced required 1 year annual visit using the CARE tools. LPA, Rosser met with Jane Uy, Executive Director, and the purpose of today's visit was explained. The facility is licensed to serve up to 25 developmentally disabled adults of which up to 5 can be non-ambulatory. Hours of operation are Mondays through Fridays 8:30AM to 1:30PM. There are currently zero (0) participant in placement . Jane stated that the facility wishes to keep the license open because in the future they plan to reopen, no tentative date for facility to reopen . The facility consists of a two-story building, located in a residential neighborhood. LPA observed a reception area, restrooms, an activity room, indoor/ outdoor activity area with a shaded space, and a parking lot. The second floor is designated for adult students (18-22) which consists of 4 classrooms of which 2 classrooms are used for non-public school students and 1 classroom are designated for adult day programming, a kitchen, 2 restrooms, an all-purpose room/conference room and a computer room.
