The inspector’s account

Licensing Program Analyst(s)(LPA) Mary Flores conducted an unannounced annual visit at the facility with focus on infection control, medication and food review. LPA Flores arrived at the facility and contacted Administrator Sonda Hickman who stated was on her way to the facility and will arrived within an hour LPA Flores met with Sonda Hickman Administrator and explained the reason for the visit.

The facility is licensed to serve (6) six ambulatory adults age range 18 through 59. The facility is located in a residential/commercial area and is a two story home which consist of 4 client rooms, 3 bathrooms, a living room, a kitchen with dining area and an attached garage area that is used as staff office and storage.

LPA Flores conducted a tour with Sonda HIckmans Administrator and observed the following:

There are currently no clients at the facility, as the licensee is waiting to be vendorize by the local Regional Center. All bedrooms are set up with all furniture, bedding, and sufficient lighting. Bathrooms are in working condition and water temperature was tested as follow; Bathroom #1(B1) 111.3 degrees F., #2(B2) tested at 105.0 degrees F., and #3(B3) tested at 109.5 degrees F. which is within the required 105 - 120 degrees F. Facility has a closet in the kitchen to store cleaning supplies and sharps with a keypad lock. and a closet in the hallway with a keypad lock to store medication and files. Non- perishables were observed during this visit. No medication or files were review as there are (0) clients at the facility. Administrator certificate was observed for Sonda Hickman #6056584735 expiration date: 6/6/23. Facility has a fire sprinkle system. No large bodies of water were observed.

COVID signs were observed in bathroom, and living room. Other infection control recommendations will be set in place upon providing care to clients.

No deficiencies were noted during this visit. Exit interview was conducted with Sonda Hickman administrator and a copy of this report was provided.
