Facility Evaluation Report
On 11/16/25 at 12:40PM, Licensing Program Analyst (LPA) Gina Saucedo arrived to conduct an unannounced, annual inspection at the facility. Upon arrival, LPA Saucedo met Chasity Taylor, Administrator.
LPA asked for the census, client, and staff files. There are no clients vendorized from Regional Center. A physical tour was conducted which is a single, story facility. The facility is cleared for six (6) ambulatory clients, ages 18 years and 59 years old.
The Kitchen area was toured. There was a refrigerator and a stove that are operable. Toxins/chemicals are locked and stored under the kitchen sink cabinet and the laundry room cabinet. The knives/sharps are also locked and inaccessible to the clients.
There is one (1) washer and dryer. The medications will be stored in a metal cabinet locked in the hallway room area and inaccessible to clients.
There is one (1) fire extinguisher in the house which is in the hallway area on your right hand side against the wall fully charged.
