Facility Evaluation Report
Licensing Program Analyst (LPA) Bernadette Allen conducted an unannounced visit to conduct an annual inspection. LPA was greeted by Precy Panlaqui Support Staff who was informed of the purpose of the visit and she allowed LPA into the facility. Natalya Trakhtenberg Administrator arrived around 10:25 AM.
The facility is licensed to serve age range 60 and over, approved for (6) non-ambulatory, of which (6) may be bedridden and approved hospice waiver for 4. The facility has 8 bedrooms, 7 bathrooms, living and dining room, kitchen. During the visit there were two (2) staff members and four (4) residents present.
At 9:35 AM LPA reviewed three (3) staff files for First Aid/CPR certification, criminal record clearance, trainings, and health screenings which appeared to be up to date.
LPA also reviewed four (4) clients’ files for admission agreements, updated physician reports, and needs and services plan which appeared to be up to date. A random audit was conducted of residents medications which appeared to be given as prescribed by their physician.
At 10:45 AM, LPA and Natalya toured the physical plant. There were no bodies of water or obstructions on the premises inside or outside. LPA inspected a total of eight (8) bedrooms and seven (7) bathrooms. The beds and bedding supplies were in good condition, adequate lighting was provided, and storage for the residents’ personal belongings was observed.
The bathrooms were in good condition and operational. The water temperature ranged from 105°F to 118. °F, and the facilities temperatures ranged from 72°F to 78°F.
