On 02/09/26 Licensing Program Analyst (LPA) Villegas conducted an unannounced Case Management visit to the facility in connection with complaint # 11-AS-20260201205407 . LPA met with Collene Rosatti as the purpose of the visit was explained.
The department determined that the facility is not in compliance with Title 22 Regulations in connection with the complaint and issued a citations for Resident Records 87506(a).
Deficiency cited under California Code of Regulation Title 22 Division 6 Chapter 8 are being cited on the attached LIC 809-D.
An exit interview was conducted, appeal rights explained, and a copy of this report was provided.
