On 11/24/25, Licensing Program Analyst (LPA) Villegas conducted a subsequent case management visit in response to Special Incident Report submitted to CCLD on 11/10/25 for incident occurring on 11/10/25; regarding Client #1 (C1). LPA met with Licensee Omobolo Adeleke as the purpose of the visit was explained.
During case management visit LPA Villegas conducted interviews with Client #3-5 (C5), and staff #2.
Due to insufficient information available at this time, the above allegations need further follow-up.
No immediate Health & Safety issues were observed.
Exit interview conducted, and a copy of this report was provided.
