Facility Evaluation Report
On 11/12/25, Licensing Program Analyst (LPA) Lizeth Villegas conducted a case management visit in response to Special Incident Report submitted to CCLD on 11/10/25 for incident occurring on 11/10/25; regarding Client #1 (C1). LPA met with Licensee Omobolo Adeleke as the purpose of the visit was explained.
During case management visit LPA Villegas conducted a Health & Safety visit which consisted of the following: tour of the physical plant and obtaining copies of the following records for C1: Full assessment dated: 07/31/2019, notes from Alcott Center For Mental Health dated: 06/17/2020 to 07/31/24, and MD note dated: 09/17/25. LPA also obtained copies of the client and staff rosters. On 11/12/25 LPA conducted interviews with Client #2 (C2). LPA unable to conduct interviews with Clients #3-5 (C3-C5) as they were out in the community.
Due to insufficient information available at this time, the above allegations need further follow-up.
No immediate Health & Safety issues were observed.
