Facility Evaluation Report
On 09/19/25, Licensing Program Analyst (LPA) Mario Leon conducted an unannounced annual required visit using the CARE Inspection Tool. LPA met with staff one, Omobola Adeleke - Administrator/Licensee (S1) and the purpose of the visit was explained. The facility is licensed to serve six (6) ambulatory only adults ages 18-59. Current census is six (6), co-ed, residents.
The facility is a single-story home located in a residential neighborhood and consist of the following: (3) client bedrooms, (2) bathrooms, living room, dining area, a kitchen, an outdoor laundry area, a shaded area with two locked cabinets used for toxins and additional storage space. The facility has board games, books, and other recreational materials for clients use.
Client bedrooms were checked, mattresses and box springs were in good condition, adequate lighting, plenty of dresser and closet space was observed. Bathroom toilets and water faucets worked properly, shower was free of mold/mildew, and there are sufficient toiletries accessible to clients. The water temperature properly measured between 105-120 F. A three (3) day supply of perishable and seven (7) day non-perishable food was observed, toxins and knifes were stored and inaccessible to clients, no weapons nor bodies of water are located on the premises and exits and walkways are free of obstruction.
LPA conducted a records review of 5 client records. LPA observed one resident without an Admission Agreement on-site. LPA also observed one resident without an Emergency ID. S1 displayed these residents' Admission Agreement and Emergency ID, electronically. Please see LIC9102TV. LPA observed 6 client Medications, no discrepancies were observed. Medications were centrally stored and properly locked, first aid kit was checked and fully stocked. The last fire was conducted on 06/06/24, 2 fire extinguisher fully charged, carbon monoxide and smoke detectors are hard-wired, tested, and all are in operational condition.
An exit interview was held with staff one, Omobola Adeleke - Administrator/Licensee (S1) and a copy of this report and Technical Violation(s) were provided.
