The inspector’s account

On 07/27/26 Licensing Program Analyst (LPA) Mario Leon made an unannounced annual inspection visit, California Department of Social Services (CDSS) met with Teresa Santacruz - Program Director and the purpose of today's visit was explained. The facility is licensed for a capacity of 21 ambulatory adults ages 18-59 of which 6 may be non-ambulatory. Per consumer rosters, there are 45 consumers who participate in the morning session with 21 on site and 24 off site; there are 42 consumers who participate in the afternoon session with 21 on site and 21 off site. Consumers are linked to the Harbor Regional center. Day program fees are current, there is an active liability insurance with an expiration date of 09/15/26.

The day program is a one-story commercial building consisting of 3 bathrooms, Lobby, Kitchen, conference room, art room, quiet room, computer room, "style" room, three (3) staff offices. CDSS observed lockers available for clients to store belongings. The day program walls and floors were in good condition, adequate lighting. Plenty of storage space, chemicals and sharps were properly locked. The restrooms were clean and within Title 22 regulations, temperature measured at 105.6°F (degrees Fahrenheit). Landline observed, CDSS observed sufficient furniture and lighting throughout the facility. Consumers bring their own lunch, however day program provides snacks if a consumer did not bring their own. The last disaster drill was conducted on 07/21/26, smoke alarms and carbon monoxide are hardwired and operational, according to the Long Beach Fire Department's visit on 07/14/26. Fire extinguishers were properly charged, and the first aid kit was available and fully stocked. There are no weapons nor bodies of water on the premises, walkways throughout the day program and all exits were clear of hazards and debris.

As a part of today's inspection LPA reviewed eight (8) consumer records and five (5) staff records. Day program does not administer medication nor handle P&I funds.

During today’s visit there has been one (1) deficiency observed, please see LIC809-D.
