On 09/08/2022 at 11:30 AM Licensing Program Analyst (LPA) Jose Calderon conducted an unannounced Annual required visit SVS Long Beach Willow Inclusion Center Facility with a primary focus on infection control measures. LPA Calderon was met by Administrator Meredith Gyetvay and the purpose of today’s visit was explained. The facility is licensed to serve 21 adult clients with behavioral issues.
There are currently 14 clients in care all clients are ambulatory. The facility is a one-story structure with (11) rooms and (3) bathrooms, activity room. kitchen and work out room, teaching rooms, computer room and staff offices.
