Facility Evaluation Report
On 05/19/2025 , Licensing Program Analyst (LPA) Deborah Lee conducted an unannounced Case Management Annual Continuation to complete Inspection started on 5/14/25. LPA met with Jovan Iglesias and explained the purpose of the visit facility is licensed to serve a total of 6 adults ages 60 and above, of which 5 may be non-ambulatory residents. There is a hospice waiver for 2. A total of 6 residents are currently residing in this facility. The Annual Licensing Fees are current.
This home consists of 4 client bedrooms and 3 client bathrooms, living room, kitchen, dining area, activity room, laundry area and patio. There were 5 residents present during the inspection.
LPA observed all required documents posted throughout the facility. The facility has a working landline telephone.
Smoke and carbon monoxide detectors were operational. There is a fully charged fire extinguisher located near front of resident. The last emergency drill was conducted on 5/2/25.
