Facility Evaluation Report

On 05/11/26 Licensing Program Analyst (LPA) Villegas conducted an unannounced Required – 1 Year Inspection and met with Program Director Nallely Gutierrez as the the purpose of the visit was explained. The facility is licensed for (18) adult clients with developmental disabilities, clients are linked to the Harbor Regional Center. Facility fees are current.

The facility is a single story building that consisting of: (3) activity rooms, kitchen, gym, (2) offices, computer room, storage room, conference room, (2) bathrooms, storage space for clients to utilize, and there are (3) outdoor shaded areas. The Facility walls and floors were in good condition, adequate lighting and sufficient furniture observed throughout the facility. Chemicals and sharps were properly locked, first aid kit was fully stocked, restrooms were clean and within Title 22 regulations. Clients bring their own meals and program provides snacks only. Water temperature measured between 105 F.- 120 F.. Land line and internet service were observed; smoke alarms and carbon monoxide are hardwired and operational. LPA observed (3) fire extinguishers were observed throughout the facility. There are no bodies nor obstructions on the premises, walkways throughout the day program and all exits were clear of hazards and debris. The last fire drill was conducted on 02/25/26, The Long Beach Fire Dept. conducted an inspection dated: 04/07/26.

LPA conducted a review of (6) client records, and (4) staff records, there are 8 clients who have medications at the day program, LPA observed that medications were safe, locked and inaccessible.
