On 08/01/2024 around 3:00 PM Licensing Program Analyst (LPA) Socorro Leandro arrived to the facility mentioned above. LPA met with staff Carmen Vasquez and explained the purpose of the visit.
Deficiency and Civil Penalty cited under California Code of Regulations, Title 22.
Deficiency regarding Acceptance and Retention Limitations for Client 1.
Client 1 was diagnosed with Alzheimer & Dementia in 2023 and then again in 2024.
An exit interview was conducted, and a copy of this report, deficiency, and civil penalty was left to staff along with their Appeal Rights.
