Facility Evaluation Report

On 04/16/2025 at 12:30pm, Licensing Program Analyst (LPA) Zina Brown conducted an unannounced annual required visit using the CARE Inspection Tool. LPA met with the Latashia Hammond Administrator and the purpose of today’s visit was explained. The facility is licensed to operate developmentally disabled clients ages 18-59 years, ambulatory only. Currently, the home has (4) clients (one of the clients is currently in the hospital and has been there since March 2025). The clients are Harbor Regional Center clients. One (1) the clients have Restricted Health Care Conditions and none are utilizing postural supports or protective devices.

The facility has a current administrator certificate (#7012434735) for Latashia Hammond valid from 11/14/2024 - 11/13/2025. The facility fees are a balance of $454 due on 06.11.2025. LPA provided pin #301187 with the option for the facility to make a payment online.

The facility is a one (1) story home located in a residential neighborhood. The property consists of the following: four (4) client bedrooms, two (2) bathrooms, a living room, a kitchen, a dining room, an office area, a laundry room, an attached garage and a outdoor shaded area.

Between the hours of 1:24p - 4:45 pm, LPA conducted a records review of (4) client records, (4) staff records, (4) clients Personal & Incidental Records, (4) Client Medication Administration Records and the facility disaster plan. The facility disaster plan was current (last conducted on 03.02.2025) and in compliance with Title 22 at the time of visit. LPA did observe a discrepancy at the time of visit.
