Facility Evaluation Report

On 02/20/26 Licensing Program Analyst (LPA) Villegas conducted an unannounced annual required visit using the CARE Inspection Tool. LPA met with Administrator as the purpose of the visit was explained. The facility is licensed to operate for (3) ambulatory or (1) may be non-ambulatory developmentally disabled adults ages 18 through 59. Clients are linked to the Harbor Regional Center. Facility fees are current.

The facility is a single-story structure located in a residential neighborhood. It consists of the following: (4) clients' rooms, (2) common bathrooms, a living area, a dining area, a kitchen, an outside patio area, and a detached garage that houses locked toxins and chemicals, emergency food and water supply, a washer and dryer, and an additional refrigerator. There are no weapons nor bodies of water on the premises, exits and walkways are free of debris/hazards. Client bedrooms were checked, mattresses and box springs were in good condition, adequate lighting, plenty of dresser and closet space was observed. Bathroom toilets and water faucets worked properly, shower was free of mold/mildew, and there are sufficient toiletries accessible to clients. The water temperature properly measured between 117.8 F.. A supply of perishable and non-perishable food was observed, sharps and knifes were stored and inaccessible to clients.

LPA conducted a records review of 4 staff records, 4 client records, 4 medication administration records, and 4 P&I ledgers, no discrepancies observed. Medications were centrally stored and properly locked, first aid kit was checked and fully stocked. The last fire was conducted on 01/14/26, 2 fire extinguisher fully charged, carbon monoxide and smoke detectors are operational. A land line was observed, surety bond is active, and the Administrator certificate is active (7036227735 exp: 03/08/27).
