Facility Evaluation Report
On 12/09/2024 at 8:40am, Licensing Program Analyst (LPA) Ronnel Agliam conducted an unannounced annual required visit (due February 2025) using the CARE Inspection Tool. LPA met with the Ronnel Agliam, Administrator and the purpose of today’s visit was explained. The facility is licensed to operate for (3) ambulatory, (1) non-ambulatory in bedroom #4 for adults ages 18 through 59. Currently, the home has 4 clients. The clients are Harbor Regional Center clients. None the clients have Restricted Health Care Conditions and none are utilizing postural supports or protective devices. Certificate of Liability Insurance is with Pennsylvania Manufactures Association Ins. Co (NAIC # 12262) - Policy # 302401 1470624 effective 07/06/2024 - 07/06/2025 with each occurrence $1,000,000/ general aggregate $3,000,000. The facility annual fee is $454 which is due on 02.01.2025 which LPA provided pin #595390 if facility choose to make facility annual payment online.
The facility is a one story home located in a residential neighborhood. The property consists of the following: 4 client bedrooms, 2 common bathrooms, staff office, living room, kitchen (110.0 F), dining room, 2 storage closet with additional food, linen, cleaning products and toiletries, an attached garage which houses an additional refrigerator washer and dryer and an outdoor shaded area.
LPA conducted a records review of (4) client records, (4) staff records, (4) clients Personal & Incidental Records and reviewed the facility disaster plan. All client & Staff records were complete. The facility disaster plan was current and in compliance with Title 22 at the time of visit. LPA reviewed (4) Client Medication Administration Records and did not observed any discrepancies at the time of visit.
At 8:45 am LPA and Markrey Madlangbayan (Direct Support Professional) toured the inside and outside of the facility. All client rooms were checked. Mattresses and box springs were in good condition, adequate lighting was observed , plenty of dresser and closet space was observed. Walls and floors were clean and in good repair. Bed linens, comforters and bath towels were adequately stocked at the time of visit. Bathrooms were found to be within Title 22 regulation. Toilets and water faucets worked properly. Shower was free of mold/mildew, there is adequate lighting, and sufficient toiletries accessible to clients. The water temperature properly measured between 105-120F ( Bathroom #1 113.6F & Bathroom #2 106.8 F).
