Facility Evaluation Report

On June 12, 2025, Licensing Program Analyst (LPA), Deborah Lee, conducted a case management visit to the facility listed above. LPA met with Program Director, Nicolas Myers, and the purpose of today's visit was explained. During today's visit (2) Clients and (3) staff were present.

LPA conducted a case management visit today to follow up on a Special Incident Report (SIR) submitted to Community Care Licensing (CCL) on 6/5/25.

During today’s visit (6/12/2025), LPA toured the facility and found that there are no Health and Safety concerns. LPA requested and obtained the following documents: Staff Roster (dated 5/27/25), Client Roster (dated 2/3/25), C1’s Placement Agency Individual Program Plan (IPP) dated 1/14/25, C1’s Individual Service Plan (dated 7/15/24) C2 Individual Service Plan (ISP) dated 7/15/24.

On 6/12/2025, LPA interviewed Program Director (A1), 2 clients (C1-2) and 3 staff (S2-S4)

There were no deficiencies cited during today’s visit.

Exit interview conducted and copy of report was provided to Program Supervisor.
