Facility Evaluation Report
On 07/14/2025 at 3:10pm, Licensing Program Analyst (LPA) Zina Brown conducted an unannounced case management - deficiencies visit and met with Robert Jakini (Administrator) and explained the purpose of the visit.
On 06/19/2025, the Department received an complaint # 11-AS-20250619142057 of which two (2) of the allegations were SUBSTANTIATED.
On 07/14/2025, civil penalties were assessed for the deficiencies cited on 07/10/2025 for complaint #11-AS-20250619142057 under the California Code of Regulation Title 22, Division 6, Chapter 8. See attached LIC 9099-D.
An exit interview was conducted with Robert Jakini, Administrator and a copy of this report was provided with appeal rights.
