Facility Evaluation Report

On 09/17/25, Licensing Program Analyst (LPA) Lizeth Villegas conducted a case management visit in response to the Special Incident Report submitted to CCLD on 09/03/25 for an incident occurring on 09/02/25; regarding staff #1 (S1) and client #1 (C1). LPA meet with Monique Weatherspoon as the purpose of the visit was explained.

On 09/17/25 LPA Villegas conducted a Health & Safety visit which consisted a tour of the physical plant, and obtaining of the following: client and staff rosters, and a copy of Client #1 records: Face Sheet, Individual Program Plan dated 12/05/2024, ABACS dated 08/25/25, discharge paperwork from Lakewood Regional Medical Center, and facility daily notes dated 09/02/25. On 09/17/25 LPA also obtained copies of the following for staff #1 (S1): Personnel record, criminal record statement, SOC341A, copy of active CPI training card, and nonviolent crisis intervention training. On 09/17/25 LPA conducted interview with staff #2-4 (S2-S4), and client #1 (C1). LPA was unable to conduct interview with S1, as S1 is no longer employed Person Centered Options II.

Due to insufficient information available at this time, the above needs further investigation.
