The inspector’s account

On 03/16/2025 at 10:15 am, the Department of Social Service Community Care Licensing Division arrived at the facility to conducted an unannounced visit to the above facility. The Department met with Jolanda Cartridge (Administrator) and discussed the purpose of the visit which is to conduct the one-year annual inspection. The facility is licensed to operate for three (3) non-ambulatory, developmentally disabled adults ages 18 through 59. Currently, the home has (3) clients. The clients are Harbor Regional Center clients. None the clients have Restricted Health Care Conditions and none are utilizing postural supports or protective devices.

The facility has a current administrator certificate for Jolanda Cartridge #6075199735 valid from 2/6/2025 - 2/5/2027. The annual fees are at a balance of $0 & have been paid in full.

The facility is a single-story structure located in a residential neighborhood which consists of the following: three (3) clients' bedrooms, two (2) common bathrooms and one (1) private bathroom (water tested at 108.5°F), a laundry room, a living area, a dining area, a kitchen, a garage and an outside patio area.

Between the hours of 10:25am - 11:30am, the Department conducted a records review for (3) client records, (4) staff records, (1) Personal & Incidental records and (3) Medication Administrator Records. All client & staff records were complete. The facility last disaster drill was conducted on 04/03/2026. The facility disaster plan is current and in compliance with Title 22 at the time of visit.

Between the hours of 11:30am - 12:25pm the Department and Jolanda Cartridge toured the inside and outside of the facility. All client rooms were checked. Mattresses and box springs were in good condition, adequate lighting was observed , plenty of dresser and closet space was observed. Walls and floors were clean and in good repair. Bed linens, comforters and bath towels were adequately stocked at the time of visit. Bathrooms were found to be within Title 22 regulation. Toilets and water faucets worked properly. Shower was free of mold/mildew, there is adequate lighting, and sufficient toiletries accessible to clients. The water temperature properly measured between 105°F-120°F( Bathroom #1: 118.8 F & Bathroom #2: 118.8°F and 114.6°F).
