Facility Evaluation Report

On 04/30/25, Licensing Program Analyst (LPA) Villegas conducted a case management visit in response to Special Incident Report submitted to CCLD on 04/21/25 for incident occurring on 4/16/25; regarding Client #1 assaulting Client #2. On 04/30/25 LPA met with Administrator Ahmed Saafir as the purpose of the visit was explained. On 04/30/25 LPA obtain copies of the following documents for client #1-2 (C1-C2); Emergency ID form, Admission agreement, Physicians report, needs and service plan, physicians psychiatry evaluation, house rules, medication list, and previous incident reports. On 04/30/25 LPA conducted interview with Administrator, and staff #1 (S1), LPA unable to interview C2 regarding the incident that occurred on 4/16/25 as C2 was out of the facility during the time of visit. LPA unable to conduct interview with C1 as C1 is currently incarcerated at Twin towers jail, C1 has a court date of 5/9/25. On 4/30/25 LPA conducted a medication review for C1 and C2, and conducted a facility tour. No immediate Health & Safety issues were observed.
