Facility Evaluation Report
On 08/07/2026, Licensing Program Analyst (LPA) Socorro Leandro conducted an unannounced Plan of Correction (POC) visit to the facility in response to a deficiency issued on 07/30/2026. LPA explained the purpose of the visit and was allowed entry to the facility.
During the visit reviewed the plan of correction that was cited on 07/30/2026. The Licensee was required to address the following citation:
During the 08/07/2026 visit, Title 22 Regulation 85088(f)(1-2) was cited for not having a signal system. As of 08/05/2026, the Licensee has made the necessary correction. On 08/07/2026, the department observed the signal systems installed in each client room.
ON 08/07/2026, a tour of the facility was conducted. Medication Administration Records (MARs) were reviewed. Food supply was reviewed. And outstanding POC's were cleared.
No civil penalties were assessed, and an exit interview was conducted with Administrator, Ahmed Saafir .
