Facility Evaluation Report
On August 1, 2026, Licensing Program Analyst (LPA) Ernand Dabuet conducted an unannounced case management visit at the facility. Upon arrival, the LPA performed a risk assessment and explained to Assistant Manager Nathan Stockard that the purpose of the visit was to conduct a health and safety inspection. At the time of the visit, there were 5 clients in Building #1 and 20 clients in Building #2.
The LPA observed two staff members present during the visit, with Stockard monitoring Building #2. A plant inspection revealed no bodies of water or obstructions on the premises. All rooms in both buildings were inspected. The fire extinguishers were charged, and both the smoke detectors and carbon monoxide detectors were operational. The hot water temperature used by clients for personal care was also checked. (Hot water temperature controls should be maintained at not less than 105 degrees Fahrenheit (40.5 degrees Celsius) and not more than 120 degrees Fahrenheit (48.8 degrees Celsius).)
The LPA observed that a 90-day supply of personal protective equipment (PPE) was adequately stocked. Additionally, a working landline and internet services were operational.
No deficiencies were cited during this visit.
An exit interview was conducted with Nathan Stockard, and a copy of the report was provided.
