Facility Evaluation Report
On August 1, 2026, Licensing Program Analyst (LPA) Ernand Dabuet conducted an unannounced Plan of Correction (POC) visit to the facility in response to a deficiency issued on July 30, 2026, during a Case Management visit. LPA Dabuet met with Nate Stockard for this visit.
During the visit, LPA Dabuet reviewed the plan of correction that was cited on July 30, 2026. The Licensee was required to address the following citation:
During the July 30, 2026 visit, Title 22 Regulation 80088(e)(1) was cited for faucets used by clients for personal care, as the water temperature was measured at 131°F in room #4 of building #2.
As of August 1, 2026, the Licensee has made the necessary correction, and the water temperature in room #4 was recorded at 105.0°F.
No civil penalties were assessed, and an exit interview was conducted with Nate Stockard.
