Facility Evaluation Report

On July 31, 2026, a virtual office meeting was held to discuss the department’s concerns regarding the facility. Attendees included Regional Manager (RM) Janae Hammond, Licensing Program Manager (LPM) Ulysses Coronel, Licensing Program Analyst (LPA) Socorro Leandro, Licensee Barbara Center, Licensee Tracy Saafir, and Administrator Ahmed Saafir.

• Signal system in both buildings is in disrepair

• No full-time staff stationed at the second building located at 1187 E. 21st Street

• Updated LIC 200 to include 1187 E. 21st Street – due by 8/8/2026

• LIC 500 Personnel Report – due by 8/30/2026

• Updated Plan of Operation – due by 8/30/2026

• LIC 215 Applicant Information for Barbara Center and Tracy Saafir – due by 8/30/2026
