The inspector’s account

On 07/30/26, Licensing Program Analysts (LPAs) Regina Cloyd and Soccoro Leandro conducted an unannounced case management visit and met with Administrator Ahmed Saafire and Staff. LPA explained the purpose of the visit. LPA toured the facility and observed the following deficiencies:

Blue Building (2103 Orange Ave.) consists of Apartment 1 & 2.

Apartment 1's hallway did not have lighting and the shower water temperature measured at 95 degrees F. The batteries in the smoke detectors need to be replaced. The signal system is inoperable. There was a hole in C13's door. C14 is missing a window screen, (clean) bed linen is needed, there is a hole in the door and wall. C15 is missing a light bulb and requires (clean) bed linen. C17 is missing a light bulb.

Brown Building (1187 E. 21st Street) consists of Apartments 3 - 6.

Apartment 3 has an inoperable signal system, hallway light is missing, and the batteries in the smoke detector needs to be changed. C1 is missing a window screen and the dresser is damaged. C2 is missing a chair and fitted sheet/bed linen. Apartment 4 has mattresses in the living room, requires light bulbs, does not have a signal system. Bathroom light was inoperable, shower unclean, and water temperature measured at 131 degrees F. C9 does not have a bedroom chair. Apartment 5 requires new batteries in the smoke detectors, floors unclean, and holes in the walls. Bathroom faucet knob damaged, unclean bathroom, and missing shower curtain. Hallway light inoperable/missing. C4's door is damage, missing (clean) bed linen, chairs, window screen and smoke detector. C5 is missing a light bulb and (clean) bed linens. Floors unclean. C6's bed linen was not clean. Continue to LIC809-C.
