On 07/15/2026, Licensing Program Analyst (LPA) Socorro Leandro conducted an unannounced case management visit. The purpose of this visit was to gather documentation.LPA met with the Licensee/Administrator, Ahmed Saafir, and the purpose of the visit was explained. LPA was granted entry to the facility.
Documents gathered consisted of Resident and Staff roster.
