Facility Evaluation Report

On 09/08/2025 at 8:15 am, Licensing Program Analyst (LPA) Zina Brown conducted an unannounced annual required visit using the CARE Inspection Tool. At LPA met with the Veronica Karen Andaya, (Administrator) and the purpose of today’s visit was explained.

The facility is licensed to operate for (4) ambulatory only adults ages 18 through 59. Currently, the home has (4) clients. The clients are from: Harbor Regional Center. None of the clients have Restricted Health Care Conditions, and are not utilizing postural supports or protective devices.

The facility has a current administrator certificate (#7004510735) for Veronica Karen Andaya (Administrator) valid from 04/02/2024 - 04/02/2026. The facility has liability insurance with Philadelphia Manufactures Association Insurance Co (NAIC# 12262) with each occurrence at $1,000,000 and general aggregate at $3,000,000 (policy #822501 1508415) effective date 01/19/2025 - 01/19/2026. The facility fees are current.

The facility is a single story home located in a residential neighborhood. The property consists of the following: three (3) bedrooms, two (2) bathrooms, an office area by the dining room, a kitchen, a living room, an attached garage with a washer and dryer, a storage of cleaning supplies(locked up in the garage and next the exit door bathroom),and an outdoor shaded area in the backyard.
